I understand that scheduling a same-day appointment with Vets ASAP Mobile Veterinary Care may incur an additional same-day service fee. This fee is separate from the standard examination and treatment fees and will be disclosed at the time of booking. I agree to pay this fee in addition to any other charges associated with my pet's visit.
I authorize Vets ASAP Mobile Veterinary Care to audio-record the physical examination of my pet(s) for the sole purpose of preparing accurate medical notes and maintaining complete veterinary records. Recordings will be used exclusively for internal medical documentation and will not be shared with third parties except as required by law or with my explicit written consent. Recordings will be retained in accordance with applicable veterinary record-keeping requirements and deleted thereafter.
I grant Vets ASAP Mobile Veterinary Care permission to photograph and/or video-record my pet(s) during appointments for use on the Vets ASAP website, social media channels (including but not limited to Instagram, Facebook, and Google), and other marketing materials. My pet's name may be used in captions. My personal contact information will never be published. This consent is voluntary and may be withdrawn at any time by notifying Vets ASAP in writing. Withdrawal of consent will not affect the quality of care provided to my pet.
I understand that Vets ASAP Mobile Veterinary Care operates on a 2–4 hour arrival window for scheduled appointments. While every effort is made to arrive as close to the start of the window as possible, mobile veterinary care involves travel between multiple locations and appointment times may vary. I agree to be available and have my pet(s) ready within the scheduled window. I understand that Vets ASAP will communicate any significant delays as promptly as possible.
I understand and agree that full payment for all veterinary services, medications, and products provided by Vets ASAP Mobile Veterinary Care is due at the time of service. Accepted payment methods include major credit cards, debit cards, and other methods as posted by Vets ASAP. I acknowledge that services rendered are subject to the fee schedule in effect at the time of my appointment. In the event that my pet requires additional diagnostics or treatments beyond the scope of the initial visit, I will be informed of associated costs before they are performed.
I authorize Vets ASAP Mobile Veterinary Care to request, receive, and share my pet's medical records with other licensed veterinary practices as necessary for the continuity of care. This includes, but is not limited to, prior vaccination history, diagnostic results, treatment records, and referral documentation. I understand that this authorization facilitates coordinated and comprehensive care for my pet. I may revoke this authorization at any time by providing written notice to Vets ASAP, and revocation will not affect disclosures already made in reliance on this consent.
